Photographer Release Form Release Statement for Videography: I, _____________________, (herein called " Videographer") hereby release full rights to reproduce and display pictures or film or photos I may have taken or art I have produced, in whole or in part, to Michael Dean. These rights shall include but not be limited to, altered images, electronic images, and all other reproductions of these images, for use on the web sites, CD-ROMs, record albums, posters, plays, movies and any other media produced and/or owned by Kittyfeet Media, Inc. or as advertisement for such media. I release Michael Dean and those acting with its authority from any responsibility, damage, or liability which may result from the taking and reproduction of said pictures; this shall include, but not be limited to, any claims for libel or invasion of privacy. Michael Dean. may use, and exploit or also not use any and all of these photos. I understand that I will not be paid in cash, and that the pleasure of contributing to the majestic art of Michael Dean is payment enough. I also understand that Michael Dean is under no obligation to use them. I will only be given a free ticket to a GWAR show. This contract is limited to footage for the movie DIY OR DIE: HOW TO SURVIVE AS AN INDEPENDENT ARTIST. By signing below I signify that I am at least 18 years of age. I also certify that I am able to contract in my own name. Michael Dean will take the tapes at the end of each session for safekeeping. Michael Dean has the right to use the images without charge, in film, Web, CD, print, and any other medium, extant or future, throughout the known (and unknown) universe. I also grant Michael Dean rights to assign these rights to a third party.
